The Godfather is my answer to the OP. I also want to say that the most underrated mafia movie would probably be Miller's Crossing IMO.

This all around. Godfather is the best, but Miller's Crossing isn't that far behind in quality.

Not classic mafia movies but often overlooked are Cronenbergs great films.
