Also did it seem like they were going to switch Irv back right there in the forest? Wouldn't his outie just turn around and see a bunch of people staring at him?

It’s possible. Also I mean you could also introduce her as Helena Eagan because in that moment it’s important she be recognized more as an Eagan.

But it’s Helena, not Helly. Helly is the one who spent all that time with Mark and formed the actual bond with him.
Helena just jumped in and took Helly’s place a few days ago and was acting as a mole for the company. Helena might have some initial feelings for Mark. But it’s clear her mission was to spy on the team and was sanctioned by the company on some level.

I was up and down some this episode having to go out to the shop and check on something. Have zero idea what’s going on now. Just going to go ahead and be done with the show.

What is the deal with the “twins” of each of the characters that we saw in this episode? What are they ?

I think my biggest question is was any of that actually “real” or are the innies just all in some virtual reality type simulation.
ETA I enjoyed the episode and the ending was obviously awesome but it was so bizarre on so many levels with a lot of wtf stuff going on that I feel like the explanation has to be something like it was a simulation

He could have been actually hurting her if it’s some elaborate VR simulation. We have seen that Lumon has crazy setups within their building. IMO everything makes much more sense if they were in some altered state somewhere in the Lumon building.

Ortbo is an anagram of robots so maybe that's it. Like the animatronic Eagans in the Perpetuity Wing.
Maybe the robot twins were used to contrast the expectation by Lumen that innies are expected to act like robots with the reality that innies are real people that will inevitably make the choice to not go along with the program.

I disagree. Aside from the major event at the end of the episode, I think innie Mark gets Helena pregnant and that plays a huge role later on.
