Probably regional specific, but always safe to assume that Burger Kang, Popeyes, Church's and McDonald's will make any list.

In this part of SC, Burger King is the worst, followed by Hardees.

Church's is the first that came to mind. Half of them seem to be in gas stations.
As far as the worst-run and dirtiest, Taco Cabana achieves this honor in Texas anyway.

I was adventurous the other day and ordered one of their salads and it was as fresh and crunchy as could be.

Burger King used to be an awesome place way back in the day, but now they've made their food/menu progressively worse, and have hired people who can't cook it.
Their fries used to be amazing, now they're some type of thick cut crap.
