I'm very ignorant here, but I'd love someone to explain how there's no system at all in place to check any specific practice billing this much. I understand that some large practices are likely billing $100s of millions but it wouldn't take long to confirm they are legit and arguably could have their own Medicare employee monitoring those types of companies.
ETA; that guy in the video maybe involved IDK, but he doesn't look like someone that has $10s of millions of dollars.

I personally know 3 doctors who went to jail for Medicare fraud in the 2018.. Amazes me how these people are getting away with this shite today...

There "technically" are systems in place but generally every fraud investigation department within our government that's job is to review any government program that gives "free" goods/services is so ridiculously understaffed and resourced that there is no realistic way they can actually do much. I've spoken with a few accountants before that had been in similar roles in different departments and it was shocking to hear some of their stories.

Which seems absurd. We can't justify a $100k accountant for every $500 mil or whatever of Medicare spent? At some point well under $500 mil it seems like someone should be auditing this?
This is where IDK the answer to big/ small government and audit.

We used to get in person audits, now we send them the info. Can fake notes, but you can't fake a physical location/practice.

This is really the reason. The cash authorized to flow through government programs increases its magnitude every year and rarely does the budget for monitoring it ever go up.
And there really is no reason the gov't wouldn't want to hire more gov't employees (they typically love it) other than the fact they really don't want the work of monitoring their programs to be done.

If people realized how easy this type of analysis is with modern data analysis overlaid with AI and systems like Palantir they’d lose their mind.
This stuff should be flagged almost instantaneously with any type of competency at the federal level but it isn’t due to incompetency and complicity from federal stakeholders.
