Corporate Email Lesson Learned: Got caught slippin', HR inbound NSFW

You young baws in the office, make sure to open all attachments before forwarding onto someone else. I was waiting on a document from a coworker that was needed hours ago to forward to a new vendor (female) for a project that's kicking off so that we can open an account. I've been pressing this guy for this certain file since this morning. He emailed me with an pdf attachment with said file name. So I hurry up and grabbed it, copied it and forwarded to said female vendor on the email chain I had started with her without looking at the attachment. As soon as I hit the send button, I got another email from coworker, that said "for real this time".
I open the original pdf attachment and see this:

I tried to recall the email, but it was too late because the lady had already opened it. I started sweating and immediately called to tell her to not open that attachment, and delete the email, and use the 2nd email. I apologized profusely, had a small heart attack, and I'm currently on oxygen as I wait on HR to bring down my pick slip due to impending sexual harassment lawsuit.
Watch out for blue falcon coworkers playing a shitty joke. Open and check all attachments before forwarding onto a client, vendor, or coworker. Send a meeting request to said coworker at the nearest Sonic location for knuckle checking seminar.

IMO the coworker who sent the file to you should be the one in trouble with HR, not you.

Sounds like you aren't doing a good job managing/reviewing the work of those under you.
You probably should have been fired a long time ago. This was just the nail in the coffin.

i was an email administrator for a time. it would be pretty easy to trace that back to the offender, fire them, and then reprimand you for not being careful.
when we first installed email archiving, it was kinda fun finding out how stupid with email could be. then you start finding affairs and other shady stuff and you don't want to go in there again until you're asked to.
