What's a good international summer trip that isn't beach centric and adjacent to a city?

Trying to plan a cheap summer trip. Solo. I have Europe coming in fall so I'm thinking central or south America or Canada. Or maybe in the US. I just am liking to try new countries.
Downside either the trip needs to be short 4 days including travel. Or I need to be close to a city with a hospital or renal clinic if longer than 4 days.
I have surgery at the end of July and will spend August recovering.
I'm barred from going above my waist in water. So will skip beach this summer.
Most mountains or canyons would be cool. Or jungle stuff. I was thinking Medellin.

I would just stick to the US if I was in your shoes. Trying to check off international passport stamps while you're in renal failure and waiting on transplant surgery is not worth the risk man. Focus on your health and safety and leave exotic trips for a later date when you have fully recovered. These places aren't going anywhere.

Vancouver checks everything you need, but isn't cheap. If you want to stay in the states but still do outdoors stuff in the same region Portland is a good option.

I like the Panama City idea, as that was a great 3-day trip for me. Spend a day in the old part of the city (architecture resembles the French Quarter at times), a day hiking in the incredible rainforest of Sobernia National Park, and of course a half day seeing the Canal and its museum.
Guatemala is also great for a quick trip: you could spend three nights between Antigua and Lake Atitlan and have a wonderful time (great in the summer because it's up in the mountains and cool).
Speaking of cool, Mexico City is a great city and a place that people never think of when discussing getting out of the heat. Puebla is another Mexican city that I'd recommend for a short trip, with an amazing colonial core.

Good call on Panama City.
I like Mexico City, but will add Merida is another good choice. Super cheap housing, great food and culture, and you could do day trip to Chichen Itza to see ruins and even a cenote too. Not hiking but still an active trip.

New Zealand (it's winter now).
Downside is that flying will be about 1/3 of your 96 hours.

Oaxaca City or San Miguel De Allende in Mexico.
Monterrey/Carmel/Big Sur area of California. Beautiful area and good hiking around Big Sur.
ETA- Just reread your post. For health reasons, I'd stay in USA.
So, maybe consider a Boston trip.
Or Portland Maine and surrounding coastal towns.

I’d also recommend Quebec City. It is amazing during the summer.

I can go to any clinic in the world and transfer treatment. Though traveling abroad would be easier on home dialysis. The thing is all of this tells me I need to enjoy life more. I can't be promised next year. So I try to do a much as I can now.
I can go anywhere with a major city. Outside the US I have to pay $320 a treatment. But if I only need one or two that's doable. Inside the US is basically free as I have insurance and already hit my out of pocket max.
It's just I've been to 43 states. Only Alaska, Hawaii, Washington, Oregon, North Dakota, Maine and New Hampshire left. And the places in the US I want to go to like southern Utah aren't really easy for me right now.

Mexico City is awesome. Great food scene there. If you want to get some salsa in check out mama rumbas. I haven’t been to Cartagena but it seems like it would fit what you are trying to do. Just be careful in Colombia.

Quebec or Maine? Acadia has nice hiking and varied views, most of which can be seen from a fairly easy walk/hike. You can drive most of the park fairly easily.
