Republicans did their best to repeal it but they were stopped by John McCain who was a republican in registration only.

McCain said he would vote for it when Trump came up with an alternative plan and warned for years he would not vote to yank people with preexisting conditions off just like that. Trump did not do what he promised (many many times) which was to come up with a new plan to replace.

John McCain was lying scum.
He was re-elected in 2016 because he swore to Arizonans he would axe Obamacare if given the chance. He had no clue who the POTUS would even be at that point, much less what alternative plan the WhiteHouse would propose.
When the opportunity presented itself, McCain said F***You to Arizona.
Any statement that his reneged promise was d/t Trump was complete bullshite!
Here's another secret, "alternative" legislation was not even Trump's to put forward. It was for McCain and Congress to put forward and for Trump to sign or veto. That's the way legislation works. That is the way Obamacare was passed.

I do health insurance. Most insurance companies follow Medicare’s pay scale. Medicare would not allow $900 for an MRI. I would think Medicare would accept about $300.

No they don't. They may follow Medicare payscales on a cost-plus basis, e.g. 150%-200% Medicare rates. But private insurance doesn't approximate CMS rates. Not even close.
