First off, McCain did not fly a fighter.
That is important because the incident originated when a "Zuni" rocket accidentally fired due to an electrical surge during power up. The Zuni was an F-4 Phantom weapon. It was not an A-4 Bomber weapon. McCain was an A-4 pilot.
The rocket struck an A-4 Bomber fuel pod. It was the plane next to McCain's own A-4. The rocket's safety mechanisms kept it from exploding. Unfortunately though, the fuel pod spewed jet fuel as it hit the ground and the rocket flames ignited it. The rest is history . . .
