If it's a small company, offer to stay longer than two weeks before leaving. You don't have to tell them everything like you're confessing to a priest, but let them know you were very happy with them and simply needed a change of job not company more or less. Always leave in such a way you wouldn't hesitate calling if you wanted to return.

Talk to them about the circumstance. They may commit to make a change and it's up to you as to whether you believe they are capable of changing. Curious question, if they have run you into the ground, why are you so attached to them?

In this instance you aren't leaving for a competitor in the same market, more pay, or to slight the current employer, it's just because you need to make a drastic change for the future of your personal health.

Once you leave you’ll realize they moved on the next day. You will be replaced immediately and they won’t ever look back. Never feel bad about leaving a company. Companies don’t give you a hug when you get home at night.

Speak for yourself. Just need to negotiate hugs into the benefits package.

Just give your notice and be professional. If you have done a good job for them and you give a 2 weeks notice why do you think it would sever ties? Life happens and for them it is on to the next.

This!!! One of my brothers had 28.5 years in with his large company, was very well respected by peers and the industry. A reduction study determined his level of staff was not really necessary. So, he and many others were severed (with 2 yrs pay I must say) to reduced employee count, etc.

I had 43 years with my large company and the corporate plan was to move all of my department to another city. Consolidation and employee reduction was the goal. I was offered a deal to be able to remain with the company but move to a temp location for 2 years until my planned retirement date. My boss had a heart. I retired with severance instead.
In short, the quote above is spot on.

Give them a very long notice and tell them you will be happy to stay as long as necessary to help train somebody to take over your responsibilities...and offer to consult even after you are gone. And then do it. Most employers understand...they were you once. As long as you are gracious to them, you won't have an issue.

This is somewhat off-topic but why are employees expected to give at least 2 weeks notice yet employers will fire and lay off with none and no severance.

I left my previous role in good grace; loved the company, people, etc. for a role that was offered to me at a 40% raise. They also hired my wife for a 20% raise.

If you truly love your job that much don’t leave. Seriously. We’re looking for the soonest out possible; were miserable and stressed 24/7. It’s not always about money man.
