Second only to a kidney stone, worst pain I've ever experienced.
I know some people are saying get to a Dr. ASAP but that didn't work for me. I went after my first night of not being able to sit or lay down before a head developed and he diagnosed me with Arthritis. A week later a head finally emerged and went back and he was able to cut it out.
Cutting it out wasn't that bad and it was instant relief, especially the first time you get to sit down. He will stuff it with gauze and tell you to pull about 2 inches out each day when in the shower. Be sure the wound is under running warm water because the gauze sticks and is painful as hell.
Your ultimate goal is to not let the hole close up before the inside is healed. Otherwise you'll be right back on the table.
Oh, I didn't mention the God awful smell when it's cut open. Be prepared!

I had one about 4 years ago. The pain was bad but not nearly as bad as the hemorrhoid surgery. I had it done at an Urgent Care and went back each day for a week to get the bandage changed. I haven't had an issue since and it only bothered me for about 2 weeks after.

Just went to the doctor about mine. I've had it for about a year. He said It didn't look too bad-I can either get the surgery or just live with it. Surgery seems like a pain.. Leaning towards living with it.
But I can't do certain exercises in the gym like bench press or sit ups, so that kind of sucks

Update: Went to the doctor this afternoon and was prescribed antibiotics. If it hasn't improved within the next two weeks he said to contact a surgeon and have it removed. I asked if the antibiotics can cure without having surgery and was told that there's a good chance considering how small the cyst is. Either way, this sucks

I feel bad for you.
I had one. I thought I had a broken tailbone and I was just going to tough it out. The thing got about the size of a ping pong ball and hurt like hell. Go to doctor, doc says we're gonna have to cut that out tomorrow evening.
Drive to doctor, put on assless gown and lay on table. The doctor and this huge black Sylvia fowels looking lady come in there with some tools and a whole lot of paper towels. I get about 5 shots around the Devil thing, and while they're working the helper girl is building this paper towel barricade around my arse.
To make a long story short, the thing exploded, it hurt like all hell, puss infused blood was everywhere, it stunk, and I had to pack gauze in it for 3 days to let this nasty yellow puss shite drain out.
Holy mother frick was it worth it though. As bad as having a wad of gauze stuffed in a hole cut at the top of your arse cheek sucks, having the cyst sucks much much worse.
GO TO THE DOCTOR TOMORROW.

You're right. Local anesthetic doesn't work on infected tissue despite what your doctor tells you. It's a whole lot easier for them just to inject a little local in their office and cut the thing out, instead of putting you on a surgical schedule. Of course you will hurt like hell thru the whole procedure.
Imo general anesthesia is the way to go. You can have a spinal for this, but it will end up taking a whole lot longer to get out of there.
