Set a game plan for the talk.

List out your shortcomings/performance issues and WHY you think those exist. Also lay out what you feel to be the best course of action to take to correct the issues. Then ask his/her opinion. Explain before they chime in that you are trying to become a more efficient and productive member of the team so you asked for this talk to ensure that happens.
