This feels like something you should probably unpack internally.
If you’re hearing “do what now?” often enough that it sends you into a character assessment of everyone who says it, maybe you mumble, speak with all the projection of a timid church mouse, you have a feminine voice, or simply aren’t saying anything worth understanding.
At some point, you have to consider the common denominator.

If I don't understand you the first time I say
"one more again" but I pronounce the again with a silent A and just "gen"

I always just used “huh?”, but may switch to “do what now?” just to spite you and your kind

I tend to judge people who say “where’s it at” or “where you at”.I cringe every time I hear someone say it but it’s become almost normal now with almost everyone adding the extra “at” to the end of a sentence. Even supposedly highly educated people.

One of my English teachers was on a tour while traveling somewhere up north. She was making conversation with another group on the tour and asked a lady where she was from. The lady replied “We’re from a place where we don’t end our sentences in prepositions.” To which she replied, “I’m sorry, where are ya’ll from, BITCH?”
