They have ankle monitors that monitor sweat for alcohol presence. Seriously.
Had a buddy get one on first offense DUI. He crashed into some parked cars downtown though.

Know a guy in college that somehow got house arrest after his first. Was only allowed to come to campus for class. Had to be home by a certain time. Guess it helped keep his record clean or something. He still couldn't drive for a few months. Most people I know just lost their license though. Just saying it's possible the judge was willing to do some weird deal.
