Sanitation systems should be higher. Clean water on tap, and shite gets swept away with a flush.

Hillary's probably not feeling the internet was a top 10 invention right about now, but Bill probably appreciates the easy access to his favorite sites (you know which ones). Al Gore really messed things up with that invention.
On a more serious note, many of the things had less efficient alternatives available at the time except antibiotics and vaccinations, for which the alternative was almost certain death, so I particularly appreciate those, along with the sanitation system.

Exactly. All the advancements that followed the printing press were standing on Gutenberg's shoulders.
However, I believe that agriculture in general (domestication of food crops and livestock animals) should be up at the top as well. None of the others happen without the advent of farming culture.

If you understand microbiology and the virulence of smallpox, completely eradicating that disease can be argued as man's single most impressive feat IMO. We have truly conquered nature so rarely in our existence and in such a grand scale.
