In an office setting? No. Never.
At a dinner setting? Dark, nice jeans with a button up, blazer, and loafers/nice boots is business casual.

Conference isn't the office, so I'd say there is a more relaxed set of rules when it comes to this. Location also matters. If this is some one day off site conference in town, dress like you'd go in to your office. If it is a multi-day at some resort or conference center out of town, and there are evening activities, you go a little more casual. Just one example, but the family tagged along to a work conference in Orlando that my wife attended a couple of years ago, and she wore joggers and jeans, and said she saw men there with Jeans and T-shirts on. I've also been down at The Grand Hotel while conferences are going on, and you would even see nice shorts and Polo shirts. But it was August and 90 degrees. No point in not being a little more comfortable since it isn't really an office setting.

Depends entirely on the circumstance.
If am specifically told to dress business casual for an event, I would never wear jeans.
If I work in an every day office setting with a 'business casual' dress code, can probably go with jeans on occasion, but you've gotta know the company culture. If you're the only worker that ever shows up in jeans, you know...read the room.
At a conference, if I'm presenting/representing my org at a booth, I wouldn't wear jeans. If I'm attending as a customer and booth-hopping for short meet and greets, sure, jeans will fly as long as they fit well and you keep it tight up top with a good company polo or sport coat.

Also depends on the purpose of the conference. If you are going to sit there for 8 hours to hear them drone on and on so you can check some sort of continuing education hours, absolutely. You can even drop the coat if it's warm enough.
If you are looking to network, talk with clients, may run into potential decision makers at companies you'd want to interview with in the future, etc.; you better step it up a notch.

Definitely Dallas. We had more than a few of the guys come in in pressed jeans, dress shirt, jacket, bolo tie, hat and boots. If they wanted to be "dressier" they would wear dark/black jeans
Our west coast office people were fine with it, but our upper east coast group thought they were dressed inappropriately. We had to tell them that it was definitely business casual in Texas. They said that to them business causal basically meant no tie.

There is no more of a "dickhead" look than a sportscoat, shirt without a tie, and blue jeans. It is an awful look.
