There are hundreds of patients with severe respiratory failure and many of them do not have access to anything above a reservoir mask. Patients above 65 or younger with comorbidities are not even assessed by ICU, I am not saying not tubed (intubated), I’m saying not assessed and no ICU staff attends when they arrest.
Staff are working as much as they can but they are starting to get sick and are emotionally overwhelmed. My friends call me in tears because they see people dying in front of them and they can only offer oxygen.

I’m not denying that Italian hospitals are overwhelmed. But why weren’t US hospitals overwhelmed with any of the last SARS-like outbreaks that have had similar numbers?

Kinda sorta.
Italy doesn’t have much primary care for whatever reason so basically everyone gets hospitalized if they’re sick.
But a healthcare system getting overwhelmed can happen to anyone in situations like this, just places with more available healthcare are less susceptible to it

I don't know the answer, but I don't think SARS or MERS were as easily transmitted. I think trusting numbers of confirmed cases with such limited testing is a bit naive, though.

If you sum the total number confirmed by all SARS and MERS outbreaks and multiply it by ten, you would just get past the halfway mark of those numbered by CoV-19 as of today.
CoV-19 is considerably less lethal then those two, but makes up for it on how contagious it is.
Most of the bad outcomes are associated with zero mitigation strategies allowing it to overwhelm the system.
Which is why the UK’s “hold my beer” Approach is odd.

He’s talking about the tipping point where the healthcare infrastructure becomes overwhelmed in certain critical path areas and they have to begin triaging access.
In this case it’s respirators that all the advanced cases need and only a limited units are available, which means patients who would’ve otherwise survived will not.
Having read about those situations in 1918 and disaster situations it’s pretty ugly to watch up close.
