Tell us about your underlying conditions. This way you'll have a record just in case they try to slip in COVID as a cause of death when it's really a blood clot or some such.

Last Sunday I woke up with the chills, body aches and fatigue. Along with some serious sinus congestion.
No fever
Went to the walk in clinic Sunday because I wasn’t feeling any better.
Chest X-ray found infection in the top of my right lung. Dr said I had an upper respiratory infection and ordered the Covid test.
