Medical Doctors really don’t like it when you call them by their first name for the most part, it’s kinda funny. I do stick to calling them doctor though because it’s kinda like being mean to your waiter/waitress, only instead of spitting in your food, they can just let you die.

I call doctors “doctor” in their professional environment, which is the only setting in which it makes sense, IMO. I’m not calling a doctor “doctor” in a personal setting, and I don’t think many of them expect you to (source: my wife who is a doctor and doesn’t give a shite about her title in personal settings).

Yeah, I'm not too impressed with a PhD in Urban Forestry from Southern University (yes they do offer a PhD in that!).
But a real Doctorate in Physics, Chemistry, Biology, or Math from a serious university has absolutely earned the title 'Doctor', more than most mere physicians have.

"Doctorates" that don't require intensive research, teaching, etc aren't real doctorates.
Graduate School has been seriously watered down.
