It's not even worth explaining. Most of the population doesn't have to be spoon-fed why the title exists, and most are happy to use the term doctor because they don't overthink it and don't have an inferiority complex that makes them feel like the doctor is insulting their intelligence by having a title.
Doctor is an educational title, not an occupation.
