Even the Black Death(bubonic plague in Europe), killing about 1/2 of the population, didn’t collapse civilization. It did however, fundamentally alter society. By that gauge, it seems it would need to kill more than half the population.

Covid is a mosquito.
It is a mosquito that will kill some.
99% of the Earths' population will NOT be killed by it.
We let the less than 1% of the population that MIGHT be at risk convince us to kill that mosquito with hand grenades.

I think a Small Pox outbreak would do us in generally. Not full collapse, but close. R Naught for SARS was way higher than COVID and its mortality rate was like 10 or 12%.
If that had escaped China, we’d probably have been fricked long ago. Or MERS which killed like 30% of those diagnosed. It’s r naught was somewhere around 0.5 though. And a lot of these diseases are infectious until symptoms show. Unlike SARS-Cov-2 which is infectious up to 48 hours before symptoms show. Supposedly.

Depends on what you mean by collapse civilization. If you mean an acute but ultimately temporary disruption to our standard of life (such as widespread disruptions to food distribution and power outages), then yes your metric for the requirements of that are correct.
If you mean a true collapse of civilization, remember that smallpox is contagious enough to be the Usain Bolt of infectious disease and carries with it a mortality of 30% and we still made it through.

Depends how weak or vulnerable the civilization is. A divided house falls a hell of a lot easier than a united one. The hyper-partisan bullshite we have going on right now lowers the threshold. I can't project a differential but logic dictates there is one and it is non-trivial.

Given the way we have reacted to Covid, it wouldn't take much. Can you imagine if something like smallpox were raging? We'd be done.

If you had the coronavirus we had now but it caused total organ failure or something, we would all be fricked.
