Lake Lure didn't, and it was warned. The Walter's dam was never, as far as I can tell, warned for imminent failure. It all stemmed from a TN state rep tweeting that it had failed. There were already Flash Flood Emergency warnings in place for it before that.
This has come directly from TVA and the dam operator.

That’s BS. Taxpayer money is going to be spent regardless, but American citizens and our country should always come first. Taxpayer money can’t stop Mother Nature, but that same money staying home VS sending millions to some 3rd World Country to study a couple monkeys frick could make a difference in the lives of many Americans. We can bitch and argue amongst ourselves about the way our taxpayer money is used, because it’s our money.

This is a nightmare. And it’s only going to get worse for those of us in E. Tn and Western, NC. Never in a million years thought it would be this bad here.

Which is good for those NC dams but there’s more on the way AND there’s the fact that TVA is going to have to empty the lakes very soon. We’re holding it for now but Douglas and others are at their limit. Those will go into the rivers that are already flooding us.

They must have a different definition of "dam failure " than I would describe.
A dam failure to me would be super catastrophic, such as an entire lake full of water inundating towns below the dam.
Maybe a failure or breach is just water going over the top of the dam.
