Does she have a business relationship with her? Is he a co-worker, a customer or a someone she buys from?
A few guesses on why he doesn't acknowledge your existence:
Could be gay, and just doesn't like straight guys.
Could be straight, and doesn't like gay guys who marry for appearance sake.
Could know something about you're past, that your wife shared with him. Examples might be: you are a wife beater, you cheated on her, you cheated with a man, etc.
Some behavior you exhibited in his presence makes you a despicable person to him.
Could be anything, but no need to worry about it.
Have you ever asked your wife about it?

As a wife, I would put a stop to it. I wouldn't allow the hug and would be a bit standoffish with someone like that. I'd be cordial but would put my husband in the mix by saying something like, "You remember so and so..." and forcing acknowledgement. You treat my husband poorly, I take that personally.

Well, my buddy who just went through this has video evidence in several secure locations of more than his wife hugging. The GPS location was paramount during divorce hearings. He controlled everything based on leverage

Assuming there is no affair be up front and non passive aggressive with it.
Had a dude that liked to swing his dick around and interrupt or ignore other guys at social functions. One time I told him to stop being a dick and acting like he’s more important than anybody else in front of his wife and other guys and he stopped. Got awkward but he apologized and shut up. We’re friends now.
Win win. A you get it off your chest and he knows how you feel. If your wife gets upset about it and defends him, sorry, but I’d press in to her about it.

Had a similar situation with one of my brothers friends when we were younger. He was really shy and really wouldnt say anything unless you spoke to him first. So after years of him walking in the house and never acknowledging anyone I started to yell hello to him before he could ever get past the kitchen. After a few times of that he would say hello to everyone before heading upstairs. Kind of interesting bc now he is a pretty talkative guy. We aactually laugh about it now and I like to think I was responsible for bringing him out of his shell.
In regard to your situation, Id def shake his hand every chance you get. Eventually he will understand that shite he was doing was extremely rude. In his slight defense though, are you positive he knows your name? Im not saying that to be an arse, but I know I absolutely hate going up to people at functions when Ive spoken to them many times but cant remember their damn name.

This is what I came to say also. The biggest problem is that your wife, who I imagine knows your concern doesn't do anything about it.

The guy is acting like he’s mad/jealous of you. Or it’s awkward being around you for a few possible reasons.
You mentioned he’s from your wife’s hometown and they knew each other prior. So it’s almost a definite they banged before and he feels awkward around you, OR they’re still banging and he feels awkward.
Just by his behavior, he banged your wife at some point.
No way a guy who has no attachment to your wife ignores you like that, when he feels comfortable hugging on her.
And what’s with not asking your wife about this situation if it’s bothering you enough to bring it to a message board for strangers to discuss?
You sound like you’re scared of the actual truth behind it all. No way I let this go by twice without saying something and you’ve gone 8 times?? Jesus, man up!
