replace the carpet,headliner, and get the seats recovered...also replace the cabin filter because you know the a/c has been pumping nicotine through it for awhile.

Just smoke about a pound of weed in there. You won't smell the cigarettes anymore nor will you care what the frick it smells like.

I saw a work truck that a guy had smoked with the window up for years in.
I never thought they'd get the smell out.
It looked like mallassas(sp) on the ceiling from tar.
I think they used Mean Green.
There was no cloth in it.Metal roof inside.So every surface was cleaned.
They got the smell out.100 %.They worked.

Rent a ozone machine and put it in the truck for a couple of days. That's what hotels use to get the smo me smell out of non smoking rooms.
Do the ozone machine after changing filter and upholstery cleaning.

Rent a carpet cleaner with hose attachment from the grocery store. Clean the seats, carpet, headliner, etc. Let it dry for a few days.

These are some good ideas to LESSEN the smell but you will never remove it unless you replace every bit of fabric and leather in the vehicle.
Step 1: clean clean clean the truck. Over and over. Especially the headliner on the drivers side. Use vinegar solution. Then a soap solution. Then a baking soda solution. Then buy a chemical cleaner as well.
Step 2: put an absorbent in it for a few days. Charcoal, cat litter, etc.
Step 3: replace the cabin filter.
Step 4: release an odor bomb inside. The best one I found was not the Ozium you see above. The best was a product I bought at Walmart in a black can. It was only $9.99. Can't remember the name.
Step 5: keep windows open as much as possible.
Over a month or so, you can release another bomb or two. The smell will be much less potent in the end but it won't go away.
