I'd have to go with New Orleans, as far as American cities go. Los Angeles is second.

This. One of the villages we visited outside Meru in Kenya was pretty filthy. We actually visited someone that invited us into their hut...that was built in the middle of a massive garbage pile. No running water, no electricity, raw sewage everywhere. Never was I more thankful that I had a small cold and couldn't smell much than that time

The best way to fully appreciate just how much LA sucks is to approach it from the backside. Drive in from the desert through Riverside. The dust particle haze and general griminess gives way to exhaust/industrial pollution haze and general griminess. When I picture LA I see a brown, dying, untrimmed palm tree obscured by the dust and fumes hanging in the air. I love San Diego, love the perfection that is the central coast, love the nor-cal redwoods and the lonely dark, Maine-like coast up there, but I loathe LA.

i took a panaramic from my hotel balcony in Anaheim with the mountains off in the distance. You could see the brown ring in the sky around the entire area.

In the US New Orleans and New York are the filthiest I have seen.
Outside the US Manila is a disgusting place. You can feel and taste the smog with every breath. Not to mention the places outside the financial district are seemingly all garbage heaps.
Bonus: While London is relatively clean for a large city, the Thames is one the most rancid smelling rivers you will ever smell outside India.
