I have had them permanently removed on both big toes. They freeze it and cut it out. It hurt after, but worth it in the long run.

Deserves to lose that toe, if one lets it get that atrocious. Plus that’s a great way to get sepsis, blood poisoning.
It’s 2022, not 1222.
There’s an urgent care on every other corner and my toe is worth $50.
That being said, I had a really bad ingrown toenail on a big toe one time. A large syringe into the big toe to numb the pain and the doc brought out the surgical shears and cut the nail down the center and plucked the whole thing out. The amount of puss and blood damned up was staggering.

I can't even imagine how bad that toe hurts in that picture. I had one on my left foot when I was in high school and it wasn't nearly that bad but the pain was unreal when someone stepped on it, or I stepped on it, etc etc.

No. If you treat them early enough (they have OTC meds that you can use, plus the ol' "pull it out" method) you won't need to go to the doc. Think I've been twice, when I was younger and didn't know how to handle them.

I'm not a Dr but I know the beginning of gangrene when I see it. That dude needs to see a Dr ASAP, like today.

Used to get them all the time in HS. One got infected and was bad so I had it cut and some sort of chemical put on the nail bed to keep it from growing back.
Not too bad, but certainly not fun at all. The initial shots to numb the toe for surgery suck pretty bad. It made my big toe feel like it was the size of my head for a minute or two before it went numb. Went home afterwards and took a nap on the couch. When I woke up a couple hours later, my toe felt like a balloon again and I could feel every single heartbeat in my toe. It felt like someone was softly hitting my toe with a hammer with every beat. Gotnthrough that night with some good painkillers and had a boot to walk around school with the next day or two.

Yes. Go to the podiatrist. Painless and you will never have another problem with ingrown. Hopefully you have it on two sides so the podiatrist can get rid of either side. You can watch the Toe Bro do this on YouTube. But it is painless. I can attest to that.

Worst pain I ever had… Doc gave me a shot in the end of my big toe, came back a few minutes later and shoved some scissors under the toenail….. it wasn’t numb.

If that pic is really your toe it's even worse than where I had bad toes as a kid and in 8th grade had partial toenail amputation where they cut off one side and killed the root so that to this day I have like 7/8ths of my left big toenail. The missing 8th is just a flat side.
BTW, the procedure is they local anesthesia inject the toe and then cut the side off and stick a long Q-tip dipped in acid to kill the root. It swells and is painful for about 3-4 days, you wear a shoe with the large toe cut out for about 2-3 weeks till it feels OK enough to go back to closed toe shoes again. Never again does that side of the toe get ingrown.

Yes. I had them on both big toes in high school. Not as bad as in the picture, but mine were very infected. Puss and blood constantly. I was playing football at the time and if someone stepped on my toe with a cleat it was the worst pain ever. It would soak my sock in blood. The flesh around the toe was swollen and gross. Occasionally I would get a podiatrist to cut the ingrown toenail out, but it would always come back. I was such a pussy and didn’t want to have the “surgery” to cut it out and permanently kill the nail root. Eventually my mom got tired of washing my bloody socks and demanded I get it done.
The worst part of the surgery was the 1 second of pain with the shot to deaden my toe. After that, I’ve never had an ingrown toenail since.
Point being…if they are bad, stop trying to fix it yourself and get a doctor to fix it permanently
