For Loomis specifically, was at some point as the bean counter, he should have put his foot down after Brees left. Like guys we are rebuilding, start that process.
A lot of other issues were on coaches and such, but as the GM you hired the coaches.

Loomis' biggest mistake was letting Payton convince him that Dennis Allen was ready for another head coaching job and could continue the same success that Payton had here.
Had this been any other situation, Loomis would have been fired with Allen, and some would say he still should have been, but since it's not, he was allowed to stay on and possibly got lucky with Moore and Shough.
I'm not going to be upset if Loomis stuck around long enough to get lucky again, but all of his other mistakes pale in comparison to giving Dennis Allen a chance to ruin this team for three years.

I'd say Marcus Davenport but I'm not sure how much of that to put on Sean Payton, so I'm gonna say Derek Carr. Trying to maintain the status quo after Drew with Carr also led to the Olave/Penning draft trades - the only way he could get Carr a potential star LT and WR since our cap was a mess. In retrospect, should've started the rebuild as soon as Jameis Winston didn't work out post-Drew. Easy to say in hindsight, though.

This is an interesting one to me. The approach to get these 2 guys is something a team trying to reload can do (which is what Mickey was trying to do, and not a team in desperate need of a rebuild does (which is what the fans wanted). But, if that trade nets us Olave and Tyler Smith*; are we as upset?
*Assuming the Saints oline coach would be smart enough to slide Tyler to LG like the Cowboys recognized.

no GM is immune to mistakes in the draft or free agency
the biggest mistake was not starting the rebuild earlier especially when you hitched your wagon to Carr who was a disaster
you hit the jackpot with Brees but failed when that era ended
hiring Allen was another huge mistake with what he put forth with the Raiders
not having a competent search was malpractice

Unger was the best player in the trade after it was finalized.
Loomis flopped on the 1st round pick however Unger kept Drew's career going. Our OL was terrible and he helped stabilize it.

The Jairus Byrd deal is the domino that started kicking the can down the road to pay for players when we couldn't pay for them up front.
Coupled with the fact that he was absolutely awful makes this a runaway.
