I've always thought that it was weird that virtually all of Star Wars seems to take place in the outer rim and almost none of it in mid or inner rim

I always thought it was weird that for 40 years, every sentient being in the galaxy had some sort of direct relationship to a runaway slave boy from Tatooine or a member of his family.

That's why the politics were so essential to setting up the empire. The prequels were poorly executed, yes, but they really built the world of Star Wars on a galactic level.

Vader isn't just a swordsmith with force powers. He has the insight and clairvoyance to rule and manage an imperial force across thousands of star systems.

Despite Vader being 100% outside of the Imperial Army/Navy command structure, mind you.

Granted, being able to choke someone in an adjacent Star Destroyer for making a stupid decision is an incredible motivator, but every so often it pays to remind folks that Vader had no proper spot in the chain of command.

Was this intentional? I know Palpatine feared Vader, nerfed him, and kept him at a distance. But I thought he was supreme commander and leader of the 501st legion.

Since they refuse to make Mara Jade canon, I suspect it's reasonable to infer that Vader was essentially operating as Hand of the Emperor.
