Starting to put together some design ideas for one. What appliances would you say are a must/highly recommended? Know a lot is personal preference, but think for starters, I think a grill, working sink, refrigerator, and pullout trash bin. Have considered a pizza oven, green egg station, etc but don’t know how much I’d use those.
Also, any other general tips or pointers for the design are greatly appreciated.

Trash bin for sure and drawers for outdoor cooking supplies ( tongs, spatulas etc..)

I would probably lean more to a griddle and a green egg instead of a grill and a green egg.

Sink? Do you really want to wash dishes outside? Unless cleaning fish or something, never understood the reason for this.

Make sure you or plumber size the pipe right to supply the BTU’s to your fixtures especially if you decide on the pizza oven.
Consider a floor drain

Would you rather walk inside every time you wash your hands while cooking?

Yeah for me, it would be hand washing as I cook a lot of raw meat and fish.

I’d go built in griddle over bbq. I use my sink all the time. Not really for washing dishes but for utensils and my hands when frying or working in the yard. If you get a fridge make sure the cutout leaves plenty of space for it to circulate air around it and/or buy a true outdoor fridge. Mine is tight and I bought a cheap arse fridge. It doesn’t keep stuff very cold and I have to defrost it every 2 months. Also make sure you have an outlet above counter for plugging in stuff and a fan on end to blow smoke away.

You have got to be trolling with this remark.

In my outdoor kitchen, I wash my dishes, utensils, etc. outside. Put them away when I'm done.
I don't drag dirty stuff back inside, clean them, then bring them back outside.
To answer OP's question, I have:
40" natural gas grill
Dual side burner
Kamado Joe station
Under-counter kegerator with dual tap tower
Three drawer cabinet
Pull out trashcan
Under-counter refrigerator
Full-size sink

Couple buddies have Blackstones and I like the idea of a grill and griddle. Wondering if I can get away with a griddle sheet you can put on top of the grill grates. I understand it wouldn’t be as effective as a Blackstone, but it would save space. Anyone tried it?

I don't really have an outdoor kitchen, just a porch with some cooking tools on it. I have a propane double burner and I use the hell out of it. I'd put a gas stove or at least a couple of gas burners on it. I have a small house so it's nice to be able to cook outside without smelling the house up. When the weather is nice I'll put the game on and cook/get drunk on the porch.

We have this. Rarely use it and use our grill all of the time. We also have a kamodo, beverage cooler, sink, and two burner side burner. All natural gas appliances.

We use our Big Green Egg an average of 3-4 times a week. Everything from brisket, ribs , hamburgers, pizza , cedar planked salmon ,drunk chicken, Thanksgiving turkey, peach cobbler ,etc. .
Add a double gas burner , and a griddle and you will be set

Ice maker. But my outdoor kitchen was actually a room attached to the house so I don't know if sn ice maker works Outside.

Whatever you cook on a grill you can cook on a griddle. You can’t reverse that though. That’s why you have the egg so you BBQ/smoke.

I put in a set of 3 drawers, and if I had it to do over, would've put in 2 sets of them.
We have a fridge, sink, access to under it, pull out trash, grill, and a 2 burner unit. The 2 burner unit is nice occasionally, but I wouldn't get it if you need the room for something else.

I have an outdoor kitchen with a roughly 15' counter on one end...the counter has a sink with access doors underneath, a beverage fridge under the counter, a combination access door/3 drawer unit, and then a blaze gas grill on top with double access doors underneath
I also have a BGE on a rolling stand with popup shelves on each side.
For me, the sink is a must have...I'm constantly in the yard and need to wash my hands, not to mention when cooking/prep/washing dishes afterwards
in fact, if I had to do it over, I'd add room for a dishwasher where I could wash all my outdoor dishes/utensils and just keep them in the dishwasher...a friend that has one recommended this and I didn't think it was a big deal until now

I just finished building mine. It's brick/iron framed with leathered black granite countertops. I went with the following:
circle cutout for my existing green egg
42" gas grill with a griddle insert
stainless sink with garbage disposal and air switch
power burner
outdoor fridge
2x garbage pullout (one for charcoal storage)
cabinets under sink
drawer and cabinet unit under grill
cabinet under burner
My kitchen is U-shaped and one full side has a 12" overhang for a bar. I wanted to maximize counter space as well and did not want clutter. I cook on my egg ~3 nights a week, but wanted a gas grill for when I came home from work later and wanted to do something a little more quickly.
I went back and forth on a gas grill/griddle, but Blaze has a good griddle insert and I talked with two different people that talked about how much of a pain it is to clean a built-in flattop if it is used often.
BBQ guys has a free design service and they are really nice to work with. I would recommend reaching out to them. Everything depends on how much money you want to spend. I've got about $25-28k into mine, but it was part of a new construction and under an existing porch line. We decided to do it under the existing porch so we didn't have to walk in the rain to an outbuilding when we eventually build one.
Here's a render. Don't have photos yet because the house isn't completely finished.
