Faux Pas Dining

What are some things that people routinely do while dining out or eating that drive you insane? This can include pet peeves related to the consumption of food itself or the dining experience as a whole.
Some of mine:
-Adding steak sauce to a steak without trying it first (or at all for that matter).
-Adding ketchup to a burger or fries before tasting them (no quality burger or fries should require ketchup).
-Ordering a steak anything besides rare, medium-rare, or medium.
-Restaurants that add the salad dressing to the salad rather than placing it on the side.
-Restaurants that put cheese on their salads without specifying on the menu.
-Ordering a second basket of chips and salsa only to ignore it once the entrees arrive.
-People who take literally 20 minutes to decide what they want to order and as a result send the waiter away multiple times.

I try to not go and eat with people who have pet peeves about what others are doing. Judgmental people are not cool to be around for something that should be enjoyable like eating out. If i even got the smallest inkling that you were consciously grading my dining habits, your arse would be at another table very quick like and walking home or to the office if we went together and I drove.

Nothing really. I am fine with people eating how they want to.
To be honest, I am way more turned off by people telling me what I should and shouldn't like/do.

I want the salad tossed in the kitchen, with a restrained amount of dressing. If the kitchen can't properly dress a salad, then what hope do they have of making decent food?

There's nothing wrong with a cold glass to me, but frosted is over the line unless it's Bud Light. Then at that point, a nice ultra cold Bud sometimes is really good since it tastes like water.

About the only thing that I remotely care about seeing other people do while eating.
Couples sitting next to each other in a booth with no one on the other side.

My goal when out to eat is typically to relax, enjoy the company I'm with, and order/eat at my own pace. If you want a fast dining experience, go to McDonalds. Of course, they put ketchup on their burgers and fries so thats out for you.
