When the attending physician told the nurse of her error, she looked surprised.

Some people who work at the same hospital said on the local subreddit that she had to bypass 4-5 safeguards to administer the drug. So if it was a mix up, it was one hell of a mix up. It may not have been intentional, but it was most definitely reckless.

How does one mix up 2 milligrams of versed and instead give 10 milligrams of Vecuronium

Except you have to then override the warning sign the Pyxsis displays. She should have known to double check at that point. You learn that shite early on. She was negligent.

She typed in the first two letters of the drug because she semi remembered the name. Unfortunately the wrong drug popped up on the screen first. This was in the local news here.
