In the first XMen movie where Wolverine and Rogue are in the subway car, Jackman had no idea that the car was going to be ripped open. His "wtf!?!" expression is legit.

At the end of filming this scene in Out of Africa (which was longer and filmed in one take), Meryl Streep started screaming and ripping her clothes off. Apparently a huge African Dung Beetle had made it's way into the inside of her dress and was moving around, I believe she started feeling it when she was getting off the wagon but powered through. The scene is first link, the dung beetle the second.

Hank Azaria has claimed that his reaction to Al Pacino's "GREAT arse!" interrogation in Heat was more a genuine reaction than any acting.
She's got a GREAT arse!
Apparently Pacino was flat for take after take and then he goes in the complete opposite direction, surprising Azaria and eliciting that great response.

Wasn't the scene in Fellowship where Aragorn was fighting the orc something similar? When the orc throws the blade at him and he knocks it out of the air with his sword, wasn't that a real blade that was thrown or something like that?

In Guardians of the Galaxy, Star Lord presents the orb whose contents could destroy the universe. He accidently drops it. The director liked it and thought it fit in well with the character so he kept it in the film

The scene where the movie theater burns down in Inglorious Basterds, the controlled fire got out of control and they ended up burning down the set, almost killing the cast and crew.

This is accurate. The stunt man (Orc) flung that blade and is was supposed to go well wide of Aragorn but went right at him and he knocked it out of the air with his sword.

I think the stunt man was supposed to throw a fake sword, but threw a real one by mistake.

the final scene in the Parking Garage episode of Seinfeld where Kramer tries to start the car and it won't start...this was definitely unscripted and you can see his castmates laughing about it in the car.

The cat on Brando's lap in the opening scene of The Godfather was a stray roaming around on the set and Brando picked it up and put it on his lap just before the scene started. Coppola liked it so much he left it in but they had to redo the audio because the cat was purring too loudly.

Thomas Mitchell playing Uncle Billy in It's a Wonderful Life. He is supposed to be drunk and stumbles out of the camera shot. A few seconds later a member of the film crew knocks over some equipment and it makes an awful racket. Suddenly you hear Mitchell from off screen yelling "I'm alright....I'm alright" as if he had done it. Jimmy Stewart cracks up a bit and Frank Capra decides to leave it in.

Also, Coppola has stated that the infamous "Leave the gun, take the cannolli" line was improvised by Richie Castellano

Another one of my favorite ones from a newer movie is in The Hateful Eight, Kurt Russell unknowingly smashes a 150 year old antique guitar. They were supposed to swap it out with a prop but he didn't know. Jennifer Jason Leigh's reaction is genuine LINK
