Unscripted film shots or accidents that were kept in the final film (other than improv)

What are some interesting unscripted scenes or shots that you know of that were left in the final cut of the film?
The "I'm walkin here" moment in Midnight Cowboy gets brought up alot. Though apparently there's some debate about how much of it was truly unscripted. LINK /
The most jaw dropping has to be Tora Tora Tora after a stunt plane loses its propeller and crashes into a bunch of parked planes, sending a bunch of stuntmen literally running for their lives.

The toe in the bottle scene in “Life” wasn’t scripted. That was all Eddie and Martin.

The T-Rex crashing through the sunroof of the jeep in Jurassic Park was a malfunction of the giant prop. The reaction you see of the kids' hands and feet going up to catch the plexiglass is 100% real. They added a couple of POV shots after because Spielberg liked the take so much.

At the beginning of Apocalypse Now, Martin Sheen punches a mirror and slices his hand on the glass.

As far as stunts go... The Road Warrior frick up is maybe the craziest ever caught in modern film:
The stuntman's legs were not supposed to catch on the car. That's not some crazy wire work... That's flipping actually happened and damn near killed the guy.
Miller left it in the movie because it's so unbeleivebale. It almost looks fake it's so insane.
The motorcycle stunt from Thx 1138 is right there, too.. Stuntman came in way too hot on the crash and then this happened:

Vic Morrow was killed, along with 2 children while they were being filmed in Twilight Zone the movie. Vic and a child were decapitated by a crashing helicopter and the helicopter landed on another child.
They left a lot of the scene in the movie.
