Biggest choke job on the golf course?

Let's hear some stories... Could be in a tournament or could be blowing a career round.
Mine:
Was competing in the 2002 USGA Junior Amateur in San Antonio.
Made it to match play as the #47 seed... Was playing the #19 seed I believe.
Had tons of college coaches following out group on the front 9 where I shot 32 and was 5up with 9 to play.
Coaches left after 9 and I started to play conservatively... Thinking I would coast in.
Made five bogies on the back nine and shot 41... Opponent made 4 birdies and shot 32. I lost 1DN when I bogeyed 18
Runner Up:
Shot 70-70 at US Amateur qualifier in 2007... Guy buried a 30 footer on 18 to tie me and send us into a playoff for the final spot. I split the fairway on the 1st hole, he pulled it left. He laid up short and was on his way to a bogey. I had 105 to the stick and made my worst swing of the day... Chunked my wedge so bad that I ended up in the creek short of the green.. I make double and lose.

Club championship when I was 18. Carried a 4 shot lead to 18 and proceeded to yank 3 left OB, stripe shot 7 down the middle, hit shot 8 to 6 feet, lip out the putt and tap in for a 10.

I was in a pretty major tournament (to me) back in 2006.
Had a 1-stroke lead on the 18th tee. Despite hitting only two fairways all day, I pulled the driver again. Only this time I missed bad and hit the roof of a hospitality tent.
I had a pretty decent lie, but I decided to be aggressive rather than just punching out into the fairway which would have given me a make-able but difficult par or at worse bogey (would have lead to a playoff. I attempted a huge slice under and around tree branches. It didn't work. The ball hit a branch and stopped 25 yards in front of me.
Now I decided to try it again and tried another big slice and plugged it deep in a bunker, I finished with a double bogey when a bogey would have landed me in a playoff."
It took me a long time to get over that mistake, I was such an idiot.

I was playing in a Pro/Caddy doubles match. I was a pretty prominent member of the club and I was playing a Construction Tycoon. We put stakes on the game, as I was pretty confident I would win. I was up at the 18th hole, and this guy decides to double the stakes. He gets hurt, and some kid takes his spot. They ended up beating me, and humiliating me in front of the club. Everybody got laid

My biggest choke job was this past year. One of my golf goals has always been to break 80 (shoot in the 70s). I was on fire, shot a 38 on the front nine and other than a couple of bad holes was doing fine on the back. Got to the box on 18 and decided to check my score. BAD IDEA. Par four and a bogey keeps me under 80. Bogey puts me at 80. I fell apart, ended up doubling the hole. I've never been so pissed to shoot an 81 in my life.

Playing in the Webb Park Mardi Gras Mania tournament last year, I bogeyed number 7 (which was my 2nd to last hole of the day) after chipping from literally 5 yards off of the green for eagle. Chipped it by about 15 feet and 3 putted for bogey. That concluded a 4 hole stretch at 5 over. Finished out of the money by a scorecard playoff.

I've blocked all of mine out from High School
My dad shot 68-68 in the City Championship to take the lead by 5 going into the second 18 on Sunday.
Shot 82 to close out the tournament.

i think we used my three shots on the short par 3 7th, but can't recall. playing those last 11 at +5 still stings.
but i have NEVER come close to being that deep in the zone before or since. dad called me after the round and told me that was the proudest he has ever been of me. that was nice, but i still felt like absolute garbage

Best round of my life is 88. Shot 38 on the front at Beaver then proceeded to cluster frick the back nine to the tune of 52. I was sick. Didn't play again for 2 months.

Playing in Tight Lies event at my home course as an Am (Got in for free as the Club Champion). During second round, was -4 thru 12 holes then proceeded to go +8 on final six holes for a smooth 76 and a missed cut by 2 shots. The last six holes include two par-3s, two par-4s under 400yds and a par five. Complete choke job.
