Female friend I know thought she had herniated a disc in the back. Went to doctor and was in surgery for heart blockages later that day.

What I bet happened was the surgeons asked for a “cardiac clearance”. The cardiologist will then do a history looking for signs of unstable heart disease such as chest pain indicative of coronary disease, unstable arrhythmias or valvular disorders. Some cardiologists will blanket order stress tests on all these pre-op patients. Sometimes this leads to heart caths to define coronary anatomy and really what they are looking for is critical coronary disease in need of revascularization prior to non-cardiac surgery, hence why it’s felt that someone needed bypass and/or valve surgery prior to back surgery. It happens often.

Friend of mine went to the doctor complaining of back pain in the left lat. come to find out, he had a heart attack.

my FIL. Technically, it was the back of his shoulder. Left me on a Thursday night complaining about shoulder pain. Went to the hospital later that night. Had open heart surgery a few days later. Passed in recovery. Never saw him again.

He went to orthopedic with lower lumbar pain and occasional fever, they said most likely pinch nerve do X ray and schedule MRI. Pain gets way worse, gets MRI and nerve block which is supposed clear pain in 2 weeks. Pain gets slightly better and no fever for a couple weeks. 3rd week after MRI the pain is as bad as its been and fever returns with labored breathing. Went to ER and was diagnosed in a day.
The pain in back was from a bacteria infection (strep), spread to heart and ate valves causing heart failure. Requiring open heart valve replacement. All of this started from a chipped tooth/ root canal 6 months ago not gum disease related.

Easy.
Goes to primary care for pre-op clearance. Since its general anesthesia, he needs pre-op labs, EKG, and chest x-ray. On EKG, they see something concerning and is sent to cardiology pre-op consult. They vind 3 vessel disease and needs surgery.

Heart pain/heart attack can present in many ways. Including back pain.

You kinda left out a pretty important piece of the puzzle in your OP.
Fever usually means infection. Infections can spread.
