The amount charged over spot has gone way up since covid. No one has offered me a valid reason why that is, so I assume it’s just because they can charge it.
To answer your question, yes you do if you sell in near term. I would assume most people buying silver are aiming for long term investment.

Coin shops often sell American gold and silver Eagles, as well as junk silver. You will pay a premium above spot price.

SD and JM bullion. SD is usually cheaper, JM ships faster.
You will pay a premium over spot and you won't recover that if you sell. So if you're looking at this as an investment, it's hard to do anything other than lose money or just break even.
If you're looking to have something of value on hand that's outside the usual fiat currency, then go for it, if that's what you want.
Just a note; if you purchase more than $10k worth, the seller is required to report your purchase to the government.
