Technically President-elect FDR was on the dais when #Shortking Giuseppe Zangara killed Chicago Mayor Anton Cermak in early 1933. It is still debated if FDR or Cermak was the actual target. Zangara was either crazy or acted crazy. He might have been a communist/anarchist or just following Al Capone's orders. Opinions vary.
2 different women, 17 days apart (1 of them a member of the Manson family - Squeaky Fromme) tried to kill Ford with handguns - Fromme's did not have a round chambered, Sarah Jane Moore did get a shot off before being restrained.
I am excluding many plots, threats, etc. that did not approach even a robust attempt (the PR nationalists assault on Blair House is close - but Truman was not there), limiting to either shots or at least possessing/producing a weapon in the President's presence and the closest any area effect attack was able to get to an attempt (the Hoover attempted bombing).
