I don't know where you live or where you work, but I can tell you with all certainty that in the greater New Orleans area, all but a small handful of people come into the ER with a payor type. The vast overwhelming majority come in with some form of insurance. If I see a "self pay" though, it's usually an illegal.

Charging people for care is reasonable, but at a certain point it starts to be counterproductive. E.g., people only come in for their cough when they're coughing up blood by which time they've been infectious for months and have given a number of other people maybe kids and pregnant women tuberculosis and nobody is much happy about this in the ER.

It won't, not even a little. The 15 states or so that provide state medicaid for illegals, will in all likelihood continue to do so, despite the Federal matching cuts. And even of these states did end medicaid for illegals, having even limited access to American Healthcare is a million times better than anything in Mexico.

For the most part they're already going to the ER for primary care. The issue is how, or if, the ER gets reimbursed for providing it.

That's a bit of a different issue, right now we're talking about federal monies going to benefit illegals. To your point though, I think there could be wiggle-room that medical facilities which render emergency aid to illegals can be reimbursed by the federal government, but only as long as they immediately report the illegal(s) as part of filing for payment for services rendered.
