Only MDs should be called doctor in all settings, IMO.
PhDs and other doctors should be called a doctor in professional settings.

Someone doesn't stop being a "doctor" because they have no license to practice. They still hold a doctorate.
If someone is an MD but they work as a CMIO of a healthcare system are they no longer a doctor?

This is almost as douchey as people who are doctors wanting to be called "doctor" outside of their professional setting.
Doctor was an educational title long before it was used to refer to physicians.

i accidentally called my latin professor at LSU "Mr." one time and he very red-facedly said "try again, i am not a MISTER"
i was 19 and didn't realize what he was getting at so i said "....madame?"

I had a middle school foreign language teacher that had a PhD. She would do get annoyed too if someone called her Mrs instead of Dr

Outside of a professional setting there is no reason to refer to anyone as doctor. All doctors(physicians or academics) put their pants on one leg at a time just like you and I do.

Even in an academic setting, I would call her by her first name. And I would mispronounce it just to piss her off.

If you hear the phrase, "is there a doctor in the house," only those who can answer the call should be called doctor.

I would NEVER introduce myself as such in public. In fact I’m always hesitant to even tell people what I do for a living.
PhD’s like the one mentioned in the OP and people like DR. Jill Biden should be mocked and not taken seriously. They are MAX Level of pretentiousness.
