Well it says she reportedly fell over and was seen by a witness. I would think if there was video they would at least make reference to it even if it's not been seen by anyone yet.

How is this carnivals fault? Ships have decks with edges, which are lined with railings. How much more do they need to do to keep you from jumping a railing to fall to your death?

If you fall over you fall over. It's much like people touring the Grand Canyon.

Well, millions upon millions of people have managed to go on a Carnival cruise without falling off the boat.

If I was a betting man, I would say the outliers who do fall off the boat fall because of their own stupidity and carelessness.

Carnival would never be found to be guilty of negligence, let alone gross negligence.
