This may be the worst way to die and the worst way to live:
College roommate driving in Houston. His wife and their three young kids are in the SUV. 18 wheeler driven by a drunk Dominican crashes into the SUV. The wife escapes the SUV unhurt. The three kids and her husband are all trapped in the SUV as it slowly burns up. Husband tells the firefighers "don't worry about me, get the kids out." They can't. Had enough time for the wife to say some prayers. There was nothing the responders could do to save the kids or the father.
This is an abridged version that omits some more gut wrenching facts.

A lot of the execution methods mentioned in this thread are why I roll my eyes when people talk about the modern world being so bad. Many of these disturbing execution methods were carried out by some of the most civilized societies of their day.
Anyway, I will contribute with two examples from my favorite movies that actually occurred offscreen:
1. Whatever Marsellus Wallace’s “hard, pipe-hittin’ n*****s” did to Zed with “a pair of pliers and a blow torch” after the pawn shop incident in Pulp Fiction.
2. How Hannibal Lecter consumed Dr. Chilton when he caught up to him shortly after the end of The Silence of the Lambs.

crucifixion
quote:
Crucifixion is a method of punishment in which the victim is nailed to a large wooden beam and left to hang days, until eventual death from exhaustion and asphyxiation.

If I'm killing myself then I want the most painful death possible since (obviously) I deserve it.

How could we ever know the answer to this? Who is going to make objective comparisons of the different possibilities?

Bleeding out internally from your heart into your chest cavity to the point your lungs can’t expand to allow you to breath then finally you go out when your heart is restricted to the point it can’t pump, which by that time there really isn’t anything left to pump anyway is a pretty bad way to go. Trust me.

In addition to the things you mentioned, an underrated aspect of crucifixion that is often overlooked is that you're completely vulnerable to the carrion birds that can casually sit on the cross beam and go to work on your ears, eyes, nose, and mouth. Getting the spear in the side was a mercy.

Sawing is worth mentioning. Reportedly it was the fate of the prophet Isaiah. You're tied upside down with legs spread. They begin sawing downward at the crotch. Even though you're losing massive amounts of blood rapidly you're still likely to be conscious until they at least get to the rib cage.

Imagine the shame and embarrassment of this poor bastard who died a few days ago.

A leading Norwegian conspiracy theorist who denied coronavirus was a dangerous disease has died from the virus after hosting two events at his property. Hans Gaarder, 60, was found to have tested positive for COVID-19 after he died from the virus on April 6, just days after he hosted an illegal gathering in Gran municipality, around 40 miles north of the Norwegian capital Oslo.

Suffocation or drowning
We get this thread every so often and I’m always amazed at how often drowning is mentioned. Sure it’d suck but come on, no one can possibly think drowning is anywhere close to being the worst way to go.
Have you ever scorched your finger on a hot pan or fire? Now imagine that feeling all over your entire body and you can’t escape it. That’s fire and that’s got to be horrible.

From near death experience. Fire.
75% of my body burned in deep 2nd to third degree burns.
The pain is excruciating and really not describable, atleast for the 2nd degree burns. Once your past to 3rd degree your nerves are damaged but there's still pain.

Horrible being buried alive sounds truly terrifying.
Painful..I'd think being brutally raped then stabbed or something like that would be right there.
