Depression. Usually leads to your other choices (Divorce, Drug Addiction, Alcoholism, lack of work ethic, etc...)

It's probably up there. Deep depression is crippling, you become another person and make irrational choices.

i'm open. i really need to start my media company

I think high achievers are definitely candidates for depression/burn out more than the average person. Reaching the pinnacle of success in academia or industry can be a drag for someone who has put all their lives into a singe focus. Its what they life for, when the challenge is gone so is their desire.

I have owned a few different businesses over the years because I get bored with them all eventually when the challenge is gone. I know it's an issue too and I can't prevent it from happening. It sucks.

I think it's particularly hard on someone who has put all their life into a career, or scholastic pursuit. I understand you have more talents and can adapt but those high achievers who have put all their life into a career can find life pretty empty when goals are achieved.
I'm too distracted by shiny objects to ever be considered a high achiever.

Yea I am no high achiever by any means, and wasn't implying I was, but I get your point because I experience it to some degree. The "fun" for me is building it but I hate maintaining it.
