I'm currently at a Fortune 500 company, if I ever spoke to the CEO I'd probably address them as Mr/Mrs. But in terms of people I would report to on a daily basis. It was on a first name basis. Companies these days don't have strict military-like hierarchy.

Depends on the job. Where I work, it's very informal from front line staff all the way to the CEO. Everyone is on a first-name basis.

mam, sir, are good signs of respect for folks I do not know. I will also hold the door open young/old, or mam/sir if I am in a position to assist easily
