Question for OT docs, PAs, NPs and Nurses (NSFW)

My friend, Phil was recently diagnosed with acute bronchitis. He's been to the doctor twice since last Thursday. He told me that he has already taken one round of antibiotics, cough suppressant (at night to help sleep - you can hear his breathing), breathing treatment, air inhaler and a steroid shot.
He went back to the doctor (different one) on Monday. She gave him a breathing treatment, told him to continue using air inhaler, cough suppressant (at night to help him sleep - you can hear his breathing), cough expectorant, and a steroid pack (to be taken over next 6 days).
His symptoms include, coughing (obviously), ears popping, congestion, dripping like a faucet (when not congested) and sneezing. Here's his question.
When he coughs, it continues to be "dry" as in not productive. At what point, if any, should he start to see any phlegm or mucus? By seeing this, would this indicate that the infection is breaking up -- and thus his coughing is then more productive? Or can it just go away without having to see the green? I told him to ask his doctor but he is very shy, similar to Punxsutawney Phil.

Has he tried mucinex? That stuff works for me every time.

I believe it initially started as a sinus infection or something similar that wasn't addressed, with the exception of taking Mucinex (OTC) and a nasal spray. This was approximately 4 to 5 weeks ago. I think it evolved into bronchitis. But I am not a doctor.

No asthma. No history of asthma. Lower 40's but has a birthday coming up, which reminds me I need to make a note on my calendar.

Gonna take a month total or so. The cough will gradually get better over time. Now take steroid pack and cough syrup to help with symptoms and wait it out.

Yes, see previous post. Went through an entire bottle (OTC).
He tells me it never "broke" up anything when he took it.
FWIW, he says his cough is "deep" inside his chest / lungs. To the point, where he aches from coughing so much --- and it continues to be dry. Even when blowing his nose, it is clear.

He needs a specialist. He might have pneumonia

It sucks that what started out as something simple --- can be such a PIA. But, by comparison to other worse infections or diseases, this is simple as well.
Just really a big inconvenience.

Is there a certain person that one would recommend in the BR area?
And for those of you who have had it, how does one diagnose pneumonia? Is the treatment the same, i.e. antibiotics and steroid pack?

The difference between pneumonia and bronchitis is on a chest xray there is a consolidation in the lung fields. Without a chest xray pneumonia cannot be diagnosed. A change in the type of cough does not necessarily correlate with pneumonia vs bronchitis.
Do you know what type antibiotic was started? Was it a macrolide or doxycycline?
