I have been to famous restaurants in Paris where the toilet is a hole in the floor where you stand above the hole on two shoe shaped foot rests. Two examples are Chez Denise (La Tour de Montlhery) and La Palette.
And also where the lady collecting the admission fee is in there the whole time acting oblivious to what you are doing (which she may or may not be).
Didn't bother me but my wife was freaked out by the hole in the floor. I think you can actually find other options but they just keep the old ones there as a novelty.

Europe has it right. They also charge to use the bathroom, which is infuriating while there but once you're inside one of them, it shows. They're usually quite clean.

I took the train to Munich almost every work day for 26 years and spent many pleasurable minutes in that place (the women's rooms are on the left when you enter). They are constantly cleaning. Of course, you pay a buck fifty. Shower costs extra.
As for the brush, the toilets have a differently-shaped bowl here and less water in them. The brush is beside every toilet and used often.

LOL. Reminds me of a book I read about WW1; a passage recounted how the Brits were building two battleships for the Turks, and when it lit off in 1914, the Brits seized the ships for the Royal Navy. But they couldn't put them into service for months. There were no toilets on board. The Turks just had the builders drill holes in the upper sloping bulkhead so the Turks could just hang their asses out and crap in the water. Turkish docks must have smelled lovely when their ships were in port.

I went to Greece on school trip over summer. Guy had to go and me too, so we pulled over at random gas station. They call it a "water closet" in Greece. He goes in and does it. I walk in to the most disgusting scene I've ever seen. First, there is no toilet, it's a square 16" by 16" x 2" deep indention into the ground with a fosset like drain. Don't think you can take dumps in it. What he did to the bathroom looked like someone dips a paint brush in green paint, and then shook it every where in the bathroom. Against walls, ground, etc. I have no idea how you can have diarreah spread 3 ft in every direction and hit the ceiling somehow, and it be leprechaun green.
My experience was European public bathrooms are horrible, even without the diarreah first. Even a crap US bathroom is better. And there are plenty stops you can make at chain gas stations that are decently nice.
