Tough to say. Lots of trust involved in that job & a heavy workload environment for everybody involved.
On my very first solo-cross country, out of a Class C airport with a whole 17 hours under my belt, they allowed me to take off on the wrong runway, into inbound traffic. True story.
In this case, a busy airport, at night, with a pilot confidently stating they have visual with traffic… not sure there was much time (or sight) to react. I could be wrong. Curious about what the investigation determines on that.

still don’t get how ATC didn’t see the CA and call a go-around. He was gonna lose separation regardless of the helo pilot maintaining visual separation.
Not really how it works. A “go around” does nothing to prevent this. If the H60 pilot reports traffic in sight and told to pass behind then separation can’t be lost from the controller side.

Almost certainly that helicopter was where it wasn't supposed to be.

From the video there was another plane taking off. One can maybe assume the helo pilot saw that plane and maybe not the CRJ. But damn I cant help as I watch this (obv not in the seat) the CRJ is extremely visible. I am not sure how he doesnt see him....

It’s clear on the audio ATC asks Pat25 if he sees him, then immediately says go behind the CRJ…45 seconds later you hear a “whooo” then another pilot says “did you see that”
Based on that, sounds like the H60 probably had visual on the wrong plane to fly behind and was blind sighted by the CRJ

Good point. Helo pilot may have been looking out the wrong window looking at the other plane thinking they have traffic in sight

The timing you’re showing is enough time for the copter to maneuver to keep separation. Helo saw the wrong aircraft or misjudged their distance.

That’s what I’m guessing. Probably saw an aircraft landing on Runway 1 and thought he was clear behind that aircraft

The landing lights of the plane are almost directly at the camera so they are very visible. At an angle, like viewed from the chopper, they are not nearly as obvious.
Still should have been visible however and the copter pilot said he had visual. Possibly looking at some other light mistaking it for the plane.

I have walked over that bridge and saw the impact point a couple years after. It was an eerily feeling. It was a case study subject at one of our recurrent training meetings.

ATC should have advised the helicopter that the RJ was on short final to the active runway, and given the helicopter a relative position to the RJ. It would be inexcusable to mistake a departing aircraft from an approaching aircraft lit up like a Christmas tree.

True, but a VFR is required to avoid all aircraft, traffic call or not.

Don’t think it was a departing aircraft…as that would have been completely on the other side of the airport moving away. I think the H60 saw another landing aircraft but further West on Runway 1 and he thought he was behind that
And it is inaudible, so can’t hear the response from PAT25 when asked if he saw the CRJ, but then ATC immediately says fly behind the CRJ, so guessing PAT25 confirmed he saw it

Supposedly all flights out of DCA cancelled tomorrow. We pushed everything to Friday.
