How does that test know that it is not just a blown head gasket?

It doesn't. I had a conversation with him. I get HG quote in the morning, they are closing up shop now.

Your engine will take a long time to get to operating temperature when sitting at idle.

If the engine is hot and the upper radiator hose is cold, the thermostat is probably not opening.

I’m assuming the heater blows hot when turned on while overheating, so it’s probably not an air pocket.

When you did the radiator was the fluid nasty looking or did any sludge come out when you drained the fluid?

Was the radiator clogged or did it spring a leak?

Back when it was replaced, everything was fine. We went on vacation with my wife's vehicle and came back to a pool of radiator fluid under my car in the driveway.

Also, tech said today hydrocarbons in the fluid are very high, bad head gasket or whatever else could cause that.

If they got a "very" positive block test it is most likely a head gasket. The other common causes are generally less likely and usually significantly more expensive.

I hope I am wrong, but a head gasket is likely an 8+ hour job on that engine so while it may be the cheapest fix it is unlikely to be cheap.

Their tester said 250 ppm on the hydrocarbons and it's supposed to be under 50. Sent me pics of the tester reading.

Definitely combustion gases in the cooling system.

I am sure they have, but you also need to check the oil to make sure it isn't milky. If it is at all milky you will want to do a second oil change after just a few miles. The tech should take care of that.
