The only reason I turn the engine off now is so the little fuel calculations can update on the dash.

Never did in the past.

I mean realistically, what could happen?

A spark? From what? You’re filling a tank of gas with a hose full of gas. A spark could cause a fire/explosion whether your car is running or not.

Hot refueling of a helicopter with j fuel is not a good plan. It is done, but only in extremis in combat.
