If it were me, I'd go to Dr Michael Silva. He's a vascular surgeon in the Houston/Galveston area. Wrote the book on carotid endarterectomies and general endovascular treatments. Also a good dude.

my question for you is this because it wasn't clear to me. Was the left side and the vertebral arteries clear during the first exam that showed 75% occlusion of the right carotid?
I find it hard to believe the right side became totally occluded with 6 weeks.

All clear on other 3 blood highways. The evidence kind of points to it not being a plaque (cholesterol) problem as evidenced by surgeon's remarks post-op and by the swiftness of blockage after surgery.

Have your doctor make an appointment for you at the Mayo Clinic in Rochester, MN. Or if need be do it yourself. Have a copy of all your records, clinical notes, lab results, surgical notes, all of it sent to the doctor there who will handle your case.

I concur with Mayo Clinic recommendation by ljhog - you need to get your doctor to get you in ASAP - and ensure ALL your medical records get up there.
You will receive a couple packets from Mayo once you are in - do everyting the packets require you to do and fill out the information requests as detailed as possible.

There are a lot of great hospitals in this country, not sure why there's a knee jerk reaction for so many people to go to Mayo or MD Anderson.

Both conditions have been discussed here, but vasculitis and hypercoagulable states need to be considered. My problem with vasculitis as the diagnosis is that vasculitis of a large artery is rarely an incidental finding. An underlying hypercoagulable state remains in the differential especially with a quick recurrence of a blockage of an artery after the endothelial injury caused by the surgery. I would ensure that anyone working up a potential hypercoagulable state has a very keen understanding of the lupus anticoagulants and how they are tested.
Please don't sit on this. You have a problem that needs to be addressed quickly.

I have personal experience with some of the staff at Mayo - my reaction is not knee jerk.

Get someone to check you out for Ehlers Danlos Syndrome Vascular type. It can be tested for by genetic test unlike normal hyper mobile type EDS.
