On a La State Police accident report

Does it clearly state who is responsible for causing the accident and liable for damages to the other vehicle?

No, it won't. And Louisiana is a comparative negligence state, meaning you can be found anywhere from 0-100% at fault.

No. Law enforcement does not determine fault. They document the incident, write a report, and let the insurance companies slug it out.
Or... if you're in Louisiana, they document the crash, attempt the find the driver who fled, and let the victim pray their insurance covers uninsured motorists.

We had one of those outside our office last week. The driver who absconded left most of his front end, but he got away somehow.

If you look at the second page for her, there is a violations section on the left midway down. Even if she wasn’t ticketed there may be a violation listed.
ETA there would be one for you too, but you should be listed with a U (I believe) for “no violations.”

They do attempt to determine who they believe was at fault for the accident, in a sense. It is separate from what may happen in a civil suit. They place "fault" by saying what the violations are of each driver. Cops do get things wrong on the reports and they don't factor in comparative fault, other than possibly placing both vehicles in violation of some issue.
