News reporting Lafayette Officer killed in Jeanerette standoff killed by "Friendly Fire"

Just saw on the news that Swat Team Officer Segus Jolivette, who was killed in the line of duty in Jeanerette during a hostage standoff, was actually killed by "friendly fire." How the hell does this happen? I'm guessing incompetent officers from multiple small-town agencies?

Kinda knew this the day it occurred, nothing official but it was an assumption, there were some things about the whole situation that came into question

Maybe poor positioning had officers in the line of fire when shite popped off. And then poor decision making firing without a clear line, combined with poor accuracy.

Very few SWAT teams are anywhere near as good/competent, on a team or individual officer level, as they believe themselves to be.

This is just dumb. The majority of swat teams go through a decent amount of training. Problem is when something goes down sometimes the training goes out the window. Obviously they don't train as much as elite military personel, but the likelihood of them mixing it up are also lower. Fight or flight is a very real thing.
What would you have them do in a situation like this? Not show up? The issue is, in a situation like this you have trained swat mixed in with a slew of even lesser trained officers.

Ah ok. I don't know much about the situation.
So person had hostages, released them, officers rushed in and the suspect opened fire? Officers fired back and we ended up in this spot?

Extremely ignorant.
Lafayette I believe has a full time swat team as many larger departments do (NOPD, BRPD, etc) so they get training rather regularly. Rumors I’d heard was the bullet went through a wall and struck him but I’m not sure if that’s accurate.
Additionally, sometimes shite happens, even in elite specialized units. You can reference the recent friendly fire incident with the seals during a training exercise if you need.
It’s a tragic situation and a definite learning opportunity to evaluate and improve tactics.

If anything, that supports what I posted. It certainly doesn’t contradict or refute it.
I have decades of gun-toting experience. CQB/CQC is tough at any level. HR of any degree is a beast of a problem.
Most people/organizations are way overconfident in their capabilities and they bite off more than they can chew.
It’s usually the result of putzing their way through what they think are “real deal” situations and coming out okay. Then the real thing hands them their arse.
It’s a very common phenomenon and unlikely to change, given the relatively low frequency of “real thing” events.

Several cowboys in this thread that are very opinionated without any real world experience on a SWAT team.
Police can do 99/100 things right, but when 1/100 things go wrong, many are critical without ever having been the man in the arena.
It appears that something went very very wrong that resulted in a tragic outcome. Does not mean that the SWAT team members are wanna be Seal team 6er’s.
Have some compassion and simply wait for the factual report to come out before casting stones. The last perfect person was hung on a cross.
Praying for all involved.

This ain't their first F up at LPD.
Back in December there were two thugs having an altercation that led to shots fired. Several bystanders were hit by LPD bullets in the crossfire.

Supposedly he walked from the CP to the bearcat to get a water and caught a stray round center mass. Can't confirm but am hearing he had taken off his vest before going for the water bottle.
